We are a group of Nurses and Caregivers that provides community-based post-hospital care for residents with complex care needs as an option for the placement of these hard-to-place cases, such as those with:

Tracheostomy

Ventilator

PEG tube

Stage 3 or 4 wounds

And others, such as those requiring residential assisted living with medical and nursing oversight.
